There was a weird mistranslation--weird because although the mistake would have been easy to make without context in a written text the translator knew the context because he would have been the one to accompany Ohtani to meet the fan if Ohtani had met the fan.

Japanese doesn't require the subject of a sentence to be stated. A more literal translation of what Ohtani said would be "The fan got talked to. The ball was retrieved." The translator assumed that Ohtani was the person who did this, and he translated as such. He was wrong. If you asked some people who could to translate the sentences out of context, they'd probably assume the person was speaking in first person as well. The issue here is that a professional translator should get clarification before make these assumptions or have some prior knowledge of likely questions like this before entering going into the interview. Translating between languages is difficult even in similar languages. Japanese and English are very different. We should give the guy some slack, but we should expect him to improve from his mistakes as well.

Not a stupid question. It can, but it is much like in any language. Usually, there is a follow up to clarify. No natural language will avoid these potential problems. It's just the languages have been practiced enough that people are generally aware how to prevent the miscommunications from happening. Any RomCom will likely have a few miscommunications caused by something not far off from this. But to anyone who hasn't tried Japanese or a language like it (not sure if there is another language that can drop subjects), but it's actually rather surprising how well it works coming from a language where it is really difficult to have this happen.
